All employees and volunteers working with, and in the presence of the St. Symphorosa School children are required to submit (usually electronically) an Archdiocese of Chicago Background check (E-Apps), attend a seminar conducted by a member of the Archdiocese (Virtus Training), complete both a Code of Conduct and CANTS form.
All of these must be completed in order to be in the presence of the children, ie. Lunch Presider, Chaperone on Field Trip, Volunteer at a School Party, etc.
1. On-Line Archdiocese Chicago Background Check (e-Apps)
a. This background check is available on-line 24/7
b. This is a one-time form that is completed
2. Virtus Training
a. A copy of your Virtus Certificate should be submitted to the school office
i. St. Symphorosa School usually holds a training session in September
b. This is a one-time training class, however there are monthly updates sent via e-mail to stay active
3. Code of Conduct Form
a. This form is to be completed and submitted to the school office
4. Child Abuse and Neglect Tracking System (CANTS) Form
a. This form is to be completed and submitted to the school office
b. The school office with send completed form to the Department of Children and Family Services (DCFS)
c. A form is to be completed each school year

Once you are on the Archdiocese of Chicago website, page down until you see a section called "Protecting Children" on the left side of the page. You should click on this section.
This will bring you to another page where you will look down the left side and see the section “Compliance for Clergy, Employees and Volunteers (Criminal Background Check/eApps, Virtus, etc.” Click on this section.
This will bring you to yet another Archdiocese of Chicago page. You will need to navigate through this page to find the all the requirements/forms by scrolling up and down on the page.
1. First Time Users need to register by creating an ID and Password. This can be done by clicking on the underlined “click here” at the top of the section.
2. “Click on the “Site Search” box and type symphorosa and then press enter.
3. Pick “St. Symphorosa School” and this will become your “primary” site (assuming that most of your volunteer work will be done here).
4. Be sure to select the “Archdiocese of Chicago” radio button.
5. Please be sure to check the “volunteer” radio button.
6. The Access Code is currently the word protection.
7. Complete the rest of the form (remember the ID and Password you chose) and click "Submit Registration".
After you have completed the registration,you will then proceed back to the login page. Once you key in your ID and Password, you should go on to complete the remainder of the required information and sections on this Background Check.
You will notice that each section, on the right side of the form, indicates a red “X”. Once you have completed each section, the red “X” will become a green “√”. All sections must have a green “√” and then and only then should the form be submitted. If all sections are not indicating a green “√” then you must go back to that section and complete it before submitting your application.
You can scroll through all of the available training sessions. If you find one close to your house and you are eligible to attend, you’ll need to register and then sign up for the class. Follow the on-line instructions. Remember to hand in a copy of your Virtus Training Certificate that is given to you at the end of the class. Remember this is a one-time class, but there are monthly e-mails that are sent to you.
